For students who are home-schooled, attending a private school, or youth who are not enrolled in school, consent forms are available for download here. Once completed, you can visit a C-TRAN Customer Service Office to receive your YOP card. For your convenience, consent forms are also available at the C-TRAN Customer Service Office.
